State police arrested 27 suspects in a drug sting Monday, including some with ties to organized crime and the Patriarca family.
Investigators said the drug operation was run out of the Decatur Square Social Club in Providence.
"This organization was running prescription drugs like a pharmacy," state police Col. Brendan Doherty said.
Investigators said they tapped the cell phone of 77-year-old Joseph Tiberi, of Coventry.
Thousands of phone calls were intercepted during the six-month investigation, dubbed Operation Club Med.
"Approximately 1,000 of them were incriminating," Capt. James Demers said.
Tiberi's brother, Richard Tiberi, was also charged.
The investigation was carried out by a joint task force of state police, Providence police, Pawtucket police and the Bureau of Alcohol, Tobacco, Firearms and Explosives.
