A driver has been charged in the death of a mother of six, killed in a three-car crash in Scituate.
David Hazard appeared in court on crutches Thursday.

WARWICK, R.I.—A judge set bail at $100,000 with surety Thursday for a man charged in a fatal three-car crash in Scituate.
David Hazard was arraigned in Kent County Court on a felony count of driving under the influence, death resulting.
Police said Hazard had been drinking and taking prescription drugs when his pickup truck crashed head-on into a car driven by Katherine O’Toole late Tuesday night on Route 12 near Route 116, killing her.
The 48-year-old Foster woman was coming home from work. She was the mother of six children.

A third car hit the pickup as it flipped off the road. The driver was not seriously injured.
Hazard was also ordered to undergo alcohol and substance abuse counseling.

God bless the O’Toole family. I’ve lived in Rhode Island all my life and I can’t believe what I have heard about this “GUY”. My mom travels that road, she is a nurse also she does private duty, I can’t imagine what this family is going through. Hazard is still out there doing whatever he wants to do and yes Im sure he will be driving very soon. There are people in jail because they got stopped for driving with a suspended license they are in jail at least until there court date I can’t understand why he is OUT!!!!!!!! What is this world coming to?????

Myjewel, did ur daughter go to the Cranston police? If she didn’t, maybe if she goes to report it now, that would count against him too…so he can get what he deserves.
First let me extend my deepest sympathies to the O’Toole family. One night late last week my daughter was driving home from school on Scituate Ave when a black truck came speeding up behind her riding her bumper. The driver proceeded to pass her so closely that he ran her into the breakdown lane and off the road. My daughter caught up to the truck and realized that she knew the truck and the person driving it (husband of co-worker) Guess who the black truck belonged to? Mr. David Hazard. My daughter went to the Scituate Police station to report the incident and when she told the officers what had happened they told her that she needed to go to Cranston PD. Evidently where he drove her off the road was Cranston not Scituate. Maybe if they had just taken the time to get a description of the vehicle and the person driving it they could have alerted Cranston PD or at least run his name in the system. Sent a car to his house. Maybe if someone had bothered to listen to a hysterical teenager they might have found out that this person has a rap sheet as long as my arm. Just maybe Mrs. O’Toole would still be here today. By the grace of god he didn’t kill my daughter that night, but now he has left a family without a mother, wife, sister or daughter. This should be a wake-up call to all police stations that when someone comes into a police station to report a dangerous driver that they should be alittle more proactive instead of passing the buck. Because maybe next time it might be one of their wives or children who won’t make it home from work or school.

I agree with the others…I don’t understand why people like this are free to roam around until they kill someone! By the way, this guy’s last name fits him…he is a F*%^king hazard!

Clearly there needs to be stiffer laws passed ASAP regarding drunk driving. Perhaps if the laws were harsher then it might deter someone from going out, getting drunk, and then getting behind the wheel of a vehicle. Now those six children have no mother. My husband’s uncle was recently hit and killed by a drunk driver. My cousin was killed by a drunk driver too. When is it going to end? I personally agree with what they did back in the old days - an eye for an eye. String ‘em up from the nearest tree!!! I bet that would reduce the amount of crime in the world and the overcrowding of our jails and prisons!!!
My thoughts and prayers are with the o’Toole Family during this difficult time. I also have NO remorse for what ever happens to this man.
My son goes to school with one of Mrs.O’Toole’s children and he and a friend were very upset that their friend has to go thru this. It has bother them very much. When you think about this kind of tragedy it goes way beyond just the Family, altho they have the brunt of the pain and heartache, this has affected the whole community, the children her kids hang out with, the family friends, and just aquaintaces through school functions that have gotten to know the family.
This man has had so many suspended licenses ( search “RI Defendant search”) why did he have a license to begin with (or did he)? Why was it not suspended permanently!! I know it may not have stopped him from driving but who knows maybe it would have. I get so angry when people drink and drive and then when they take a life it infuriates me!!! I wish our government would govern with an eye for eye a tooth for a tooth sometimes. I hope this guy rots in jail!!! To the children of Mrs. O’Toole….. remember your mom will always be at your side.
Mr. O’Toole I can only imagine the pain and emptiness you feel. Hopefully time will heal your pain and you will remember all the good times the 2 of you have had. Its those memories that will make you all smile again.
